[摘要] 意图 探討早产儿肺通明膜病经肺泡表面活性物质+鼻塞式继续气道正压通气医治的作用。 办法 选取本院2016年11月~2017年11月收治的肺通明膜病早产儿68例,根据不同医治办法分为两组,32例施予鼻塞式继续气道正压通气医治者为对照组,36例予以肺泡表面活性物质+鼻塞式继续气道正压通气医治者为实验组,回忆性比照剖析两组血气剖析目标、血浆白介素细胞6(interleukin-6,IL-6)、白介素细胞8(interleukin-8,IL-8)与并发症发作状况。 成果 医治后,实验组血氧饱和度(percutaneous oxygen saturation,SpO2)、血氧分压(partial pressure of oxygen,PaO2)、二氧化碳分压(partial pressure of carbon dioxide,PaCO2)、氢离子浓度指数(hydrogen ion concentration,pH)优于对照组,且实验组IL-6、IL-8均较对照组低,差异有统计学含义(P<0.05);实验组并发症发作率为5.56%,较对照组的21.88%低,差异有统计学含义(P<0.05)。 定论 经过肺泡表面活性物质+鼻塞式继续气道正压通气医治早产儿肺通明病可有用改进其血气剖析目标,并下降IL-6、IL-8水平,到达削减并发症、改进预后的作用,值得推行。

Application of hyaline membrane disease in premature infants

[Abstract] Objective To investigate the effect of pulmonary surfactant membrane combined with nasal continuous positive airway pressure ventilation in preterm infants with hyaline membrane disease. Methods A total of 68 preterm infants with hyaline membrane disease treated in our hospital from November 2016 to November 2017 were selected and divided into two groups according to different treatment methods. 32 patients undergoing nasal continuous positive airway pressure were set as the control group.36 patients treated with alveolar surfactant+nasal continuous positive airway pressure ventilation were selected as the experimental group. The blood gas analysis index, plasma interleukin 6 (IL-6), Interleukin-8 (IL-8) and the incidence of complications between the two groups were retrospectively compared and analyzed. Results After the treatment, the percutaneous oxygen saturation (SpO2), partial pressure of oxygen (PaO2), partial pressure of carbon dioxide (PaCO2), hydrogen ion concentration index (PH) in the experimental group were better than those of the control group. The level of IL-6 and IL-8 in the experimental group was lower than that of the control group, and the difference was significant(P<0.05). The incidence of complications in the experimental group was 5.56%, which was significantly lower than that in the control group(P<0.05). Conclusion Alveolar surfactant combined with nasal continuous positive airway pressure in the treatment of hyaline membrane disease can effectively improve the blood gas analysis index and reduce the IL-6, IL-8 levels, to reduce the complications and improve the effect of the prognosis, which is worth promoting.
